Creates an app for the authenticated client and returns the id to send as the app-id header on every messaging request from then on.
Authenticates with api-token alone — do not send an app-id header. Requiring one here would be circular, since this endpoint is how an app-id comes to exist.
The name must be unique among your active apps, which is the only condition that answers 409. There is no cap on how many apps a client may hold.

Create an app with POST /v1/apps by sending a unique name and the api-token header. Do not send an app-id header: the response returns the app's id, which you use as app-id on subsequent messaging requests.
